Vaccination clinic June 1

Lehigh Valley Health Network is partnering with The Swain School, 1100 S. 24th St., Allentown, to provide a community COVID-19 vaccination clinic 4-7 p.m. June 1. The clinic will be open to anyone 12 years of age and older. The clinic will be held in the McCann Center; signs will be posted at the appropriate entrance.

The Pfizer vaccine will be administered and is free of charge. Those receiving a vaccination should fill out a consent form and review the EUA Fact Sheet ahead of time. Anyone under the age of 18 will need to be accompanied by a parent/guardian to receive the vaccination.

Preregistration is required to secure a vaccine. Registration can be done by clicking https://docs.google.com/forms/d/e/1FAIpQLSduA_dY-_LQJebnZEUszLyi_cdrbkLKoyEfQJNF0QQHhYYXOg/viewform. Walk-in’s are welcome as long as supplies are available. If a parent/guardian is registering a child, the child’s name and demographics should be used. The Pfizer vaccine is a two-dose vaccine. The second inoculation will occur at The Swain School 4-7 p.m. June 22.
