Carol A. Smith, 57, from Allentown, died on Sept. 26, 2018, in ManorCare Health Services, Allentown.

Born in Allentown, she was the daughter of the late Charles and Marlene (Erb) Souders.

She was a payor log analyst at Community Health Systems, Bethlehem, for many years.

Her survivors include a son Zachary R. Smith and a brother Charles Souders.

Funeral services will be private.
