Patricia M. Holencik, 57, of Cornelius, Ore., and formerly of Northampton and Staten Island, N.Y., died Sept. 8, 2016, at Tuality Community Hospital. She was the wife of Eric Holencik for 36 years.

Born in New York, N.Y., she was a daughter of the late John H. and Virginia (Smith) Creel.

She graduated from Northampton High School. She attended Northampton County Community College for three years.

She was a system coordinator at CareOregon, Portland, Ore. She formerly worked at Pacificare in Hillsboro, Ore.

She was a member of St. Alexander’s Catholic Church, Cornelius, Ore.

In addition to her husband, she is survived by a son, Casey and his wife Hope of Mossyrock, Wash.; two daughters, Erica and Kelly, both at home; two sons, Jonathan and Hunter and daughter Julia of Ohio; four grandchildren, Finn, Rose, Caleb and Elias; four sisters, Kerry and her husband Dan Slotkoff of Doylestown, Helen and husband Al Weichman of Reeders, Moira Creel of Tannersville and Peggy and her husband Paul Marinelli of North Catasauqua; a brother, Kevin and his wife Joanne Creel of Fleetwood; a sister-in-law, Gloria Creel of Virginia; an uncle, Edward and his wife Barbara Smith of Georgia; two aunts, Charlotte Zopfi of Washington and Joan Smith of Long Island, N.Y.; a father-in-law Alois Holencik of Coplay; cousins; nieces; nephews; and a brother-in-law and sisters-in-law and their spouses.

She was predeceased by a brother, John.

Contributions may be made to St. Jude Children’s Research Hospital, American Lung Association or Bonnie L. Hays Small Animal Shelter.

A celebration of life service will be scheduled for the beginning of 2017.
