Theodore Kozak, 89, of Bethlehem, died Nov. 24, 2015, at his home. Born in Bethlehem, he was a son of the late Wasyl and Sophie (Radon) Kozak.
He was in the U.S. Army during World War II.
He was a machinist for 42 years at Bethlehem Steel.
He was a member of St. Josaphat Ukrainian Catholic Church.
He is survived by a niece, Barbara Kozak of Bethlehem; three nephews, John Kozak Jr. of East Greenville and Raymond Kozak and Daniel Kozak, both of Bethlehem; grand-nieces; grand-nephews; great-nieces; and great-nephews.
He was predeceased by two brothers, Michael Kozak and John Kozak Sr.; and two sisters, Anna and Mary Kozak.
